How can I get a copy of my medical records?

Is there a charge for a copy of my medical records?

  • Generally, we do not charge a patient for a copy of his or her medical record.  However, if you ask for multiple copies of the same records, there may be a charge after the first copy.  If it has been an extended period fo time since you were seen in our office and your records are in off-site storage, there may also be a charge to cover our costs of retrieving the file from off-site.  Our charges are the amounts permitted by Maryland law.
  • There is also no charge for records sent to another physician as part of your treatment or coordination of care, or for records sent to your health insurance company in order for them to process a claim for our services.
  • There will be a charge for medical records sent to other parties such as life insurance companies or attorneys.  Our charges are the amounts permitted by Maryland law.

How can my primary care physician (PCP) or other physician get a copy of my medical record?

  • We routinely send copies of your diagnostic test results and office notes to your PCP, as permitted by Federal HIPAA regulations for treatment and coordination of care.  If you wish your records sent to another physician, you will need to sign an Authorization to Release Medical Information. 

I need a copy of my hospital test results.  Can you give me a copy from your file?

  • In accordance with Maryland medical records law and our office policy, we do not release medical records from other providers.  You will need to contact those other providers directly to obtain copies of such records.  However, if we ordered or performed the hospital test or procedure, we can release a copy of the test results or procedure report to the patient.
